Submitting a Claim

Filing a claim for PAWS to process is simple and easy! To help everything go quickly and smoothly, follow these steps:

1. Simply visit a licensed veterinarian.
2. Pay for the visit at the time of service.
3. Gather up the following documents:
   • A copy of your Animal Limited Warranty that was given to you when you purchased your animal, completed and signed;
   • All veterinary documents, including diagnosis of illness and an itemized statement of treatment and costs; and
   • If your animal was shipped to you, a copy of the health guarantee.
4. Send all of your documents to us.

Please use only one method. Duplicate claim submissions may delay processing of your claim.

PLEASE REMEMBER

• You must pay all of your veterinary fees in full at the time of visit - your warranty is for reimbursement of these fees.
• Remember to submit all of your receipts and that the statement from your veterinarian shows the diagnosis and the treatment and itemizes all the costs.
• Don't forget to include a copy of your Animal Limited Warranty. We must have this document in order to ensure coverage and process your claim.
• There may be time limits for submitting a claim. Check your Animal Limited Warranty to make sure you meet any deadlines.

NOTE: Making a false or fraudulent claim may be a crime. Violators may be subject to criminal prosecution.
